Your list should include the name of the medication that you are taking, the dosage, and how often you take it. You should also include any supplements, or over the counter medications, you take.

Primary Care Physician's Information

Your eye doctor will need to have your primary care physician's contact information on file. You will need to give them your doctor's name, address, and phone number.

Wear Your Contacts, Bring Your Glasses
